When I pick up stitches for the neck I always get holes. What do I do to prevent it?
You can pick up extra stitches and then in the second roe decrease them. Also make sure that you twist the stitches that you pick up in the gaps.

I finished the neck following the pattern and can't get my head through the opening. Is there any way of fixing it?
You probably cast off to tightly. Undo the cast off take a much larger needle and cast off again.When you slip stitch the neck in place make sure you do it very loosely.

Is there a formula to pick up stitches for a buttonhole band?
For basic yarns pick up three stitches out of 4 rows if the band is done in ribbing, for a band in garter stitch only pick up 2 stitches out of 4 rows. For any specialty yarn check the tension and you will be able to figure it out.
